Please list any breed, weight or other pet restrictions for your home. If you are aware of any deposits needed or monthly fees, or if you need to submit a photo for approval prior to adopting please let us know here. If you are a renter, you must include your landlord's contact information to move forward in the process.
Please let us know how long you are away from your home on any given day, and how long your pet will be left alone or crated.
Please provide the ages and relationship of all people living in your home.

Please list any expected major life changes you and/or your family are expecting in the next 5-10 years. Some examples include moving, having children, changing careers etc.
Current & Past Pet Information
Please provide the name and age of all current pets living in your home. This includes reptiles, birds, rodents or other types of animals. Also include any pets living in your home belonging to a roommate or family member. You must include your vet's contact information to move forward in the process.
If in the past 5 years you have owned other pets, please provide their names and what happened to them. If you have rehomed/returned a pet to another rescue or shelter, please provide both the pet name and the organization name. Please also note if you have ever declawed a cat or adopted a declawed cat.

Training Plans
If your pet will need to be trained as either a service dog or emotional support animal, please select type of training here and plan to discuss during the screening. Please note that Lucky Dog cannot guarantee any particular pet will be appropriate as an ESA or therapy pet.

Terms and Conditions

Lucky Dog's rescued animals must be indoor animals only and must be spayed or neutered. Lucky Dog does not permit the use of electric fences. Lucky Dog must be contacted if the adoption does not work out for any reason. Applicants must be at least 21 years of age. By checking this box you acknowledge and accept these terms of adoption.
